Should the inner bearing have set screws installed? I bought mine used and just noticed no set screws. What kind of problem would this create if not screwed in? I do notice when turning my axle the chain changes tension on certain points of the rotation and wonder if this may be the cause.

don't be too worried about the grub/set screws in the inner bearing. Most karters will leave them out as the outside ones are sufficient to hold the axle in place meaning less to undo when you change an axle, less marks created in the axle allowing smoother axle changes, and potentially less drag.
You should also note that when you have the third bearing (middle one in the chassis) that it is an option to run grub screws in that bearing and the right bearing, leaving the left side loose, sometimes karters find that this can help to free up the rear end and stop the axle binding up off the exit of turns, helping the driver to get the power down without the engine bogging and drive away smoother and faster.
